WRI is partnering with Cooper University Health Care as the evaluator on its Integrative Health Care for Veterans, Military, and First Responders Program. Established through a grant by the NJ Department of Health, this program expands the quality of care provided to members of the military through HeroCare ConnectTM― a partnership with Deborah Heart and Lung Center― to better serve active duty military, Veterans, and family members of any current or former military members across southern New Jersey. The program focuses on the integration of care between primary care, behavioral health, palliative and hospice care through priority access to care and navigation of services. As the evaluator, WRI will be working closely with program staff, participants and stakeholders to collect a comprehensive assessment of progress against program goals related to timely care access and navigation, patient placement, community education and outreach, as well as outline its impact on social determinants of health. This data will provide critical information for improving program outcomes. The evaluation is expected to conclude in early Spring of 2020. Learn more.
