Integrated Health Care is a program resulting from Deborah Heart and Lung Center and Cooper University Health Care’s partnership HeroCare Connect (which focuses on veterans, active duty, and retired military and their families) and their independent programs supporting first-responders. Since December 2019, WRI has evaluated this program for efficacy and overall impact by conducting interviews with key staff members from both organizations on marketing, program management, and patient navigation. Additionally, WRI analyzed responses to customer satisfaction surveys conducted after provider appointments to assess patient satisfaction. The program was successful in meeting goals related to appointment scheduling and assessments thanks to their significant efforts in community outreach, including several educational sessions at the Joint Base McGuire-Dix Lakehurst and additional first-responder locations. An extension of the program’s funding by the Department of Health will enable Deborah Heart and Lung Center and Cooper University Health Care to continue to improve program processes and outcomes, including ways to enhance access to referrals to services and better track metrics on their impact on social determinants of health.
